UMC is hosting a drive-thru COVID-19 vaccination clinic this Friday in our headquarters office parking lot. Thank you to SMACNA Western Washington for helping organize this clinic for UMC.
When: Friday, May 14th (1st dose) | Friday, June 4th (2nd dose)
Where: UMC HQ Parking Lot, 11611 49th Pl W, Mukilteo, WA 98275
Which Vaccine? Pfizer (Must have the same brand for both vaccinations.)
Food? Yes! Goodies and other snacks will also be served!

What you need to know:
All participants need to fill out the consent form and billing form along with a photocopy of their ID and insurance card. There will be a copy of the consent and billing form linked to the confirmation email once the appointment is created.
